Parking

Free Valet Parking is available at the Front Entrance, which faces Jack Martin Boulevard

Free Parking is also available in the parking garage on the Route 88 side of the medical center

Handicap parking is available in Parking Lot C, outside the main entrance facing

Registration Information for Pre-Admission Testing
Please arrive 30 minutes prior to appointment time
What to Bring:

Prescription / Order from Physician that indicates the test you are having and your diagnosis

Insurance Cards / Proof of Address

Driver's License / Proof of Identity
-
Referral Form / Authorization Number. It is the responsibility of the patient to provide the referral to the hospital.

Health/Medication History Form - Please Print Out & Bring With You
For Workers Compensation or Motor Vehicle Accidents, please bring:

Claim Number

Insurance Name, Address, and Phone Number

Know your Insurance Coverage:

To help speed up the process, please check with your insurance company to be sure that we are the correct in-network facility for your desired test. Make sure to ask if a co-payment will be required. It is important to check with your insurance provider to see if you will need a referral and/or authorization number for all tests
Payment:
- If required by your insurance, you will be responsible to make a co-payment at time of service
- If you have no insurance coverage, payment for services is due at time of registration
- Please contact a financial counselor at 732-840-3340 for information on payment optiont
- Payment may be made by Cash, Personal Check or by one of the following credit cards: Visa, MasterCard, American Express, or Discover
Requirements for Surgery

Keep Pre-Admission Testing Appointment

Follow through with medical clearance

If illness occurs prior to surgery, report to your physician

Have a reliable adult available for transportation home, and for 24-hour companionship after your surgery

Call the day before your surgery for the exact arrival time, 732-836-4378 (between 2:00 p.m. and 7:00 p.m.
Medications to Avoid (7-10 days prior to surgical date)
- Vitamin E
- Fish Oil
- Herbal Preparations (i.e. Ginkgo, Ginsing, St. John's Wart)
- Vitamin Supplements
- Check with your physician regarding Coumadin, Plavix, Aspirin, or any blood thinners (i.e. Advil, Motrin, Aleve, Excedrin)
Day of Surgery — Requirements

No foods or liquids after midnight prior to surgical day
-
No lotions, body creams, or alcohol-based products (i.e. hair spray, after shave, mouse)

No makeup
-
No jewelry or body piercing
-
Refrain from smoking and drinking alcohol
-
No contact lenses
-
No bobby pins
-
No tampons
-
Leave valuables at home
-
You do not have to remove nail polish
-
Showering the morning of surgery is permitted
-
You may brush your teeth and use mouthwash but avoid swallowing any water or mouthwash
-
Wear comfortable, non-constricting clothing

Dentures are allowed. We will supply a container.

Reading material, music, or any form of relaxation you enjoy

Area may be chilly, so you may want a sweatshirt or sweater

Do not take diabetic medication morning of surgery (unless otherwise instructed by your doctor)

Do not take insulin morning of surgery

Do check blood sugar morning of surgery, if possible
Day of Surgery — What To Expect

Arrival time is 1½ hours prior to your surgical time

Check in at Surgical Day Stay waiting area (2nd floor)

Try and Relax. The nurses know you are there

Before using the restroom, please check with a nurse to see if any specimens are needed
