Are you at risk for Medication-Related Problems?
Do you...
- Take several different medications?
- Take medications for different conditions or illnesses?
- Take non-prescription medications, herbals, vitamins, or supplements in addition to prescription medications?
If you or a loved one answered yes to any of the above questions, you may benefit from a medication consultation with our Senior Care Consultant Pharmacist.
Download our RxCheck Questionnaire to see if you are at risk.
For those patients who take multiple medications, our clinical pharmacist specialist is available for private consultations and medication therapy management. The pharmacist will assess your current medications for optimal therapy, help minimize or eliminate unnecessary drugs if needed, and help reduce your risk for medication-related side effects and interactions. More specifically a consultant pharmacist can:
- Identify and reduce medication-related problems such as side effects and drug interactions
- Help identify and eliminate unnecessary medications, simplify taking medications, and reduce medication costs
- Recommend appropriate packaging and dosage forms for easier use of medications
- Serve as your advocate with your physicians to ensure the use of the most appropriate medications
- Provide you with a Personal Medication Record and Written Action Plan
